If you wish to cancel or reschedule your appointment, kindly provide us with 1 business days advance notice or a $150.00″ late cancellation” or “no show” fee will be applied before the next appointment will be offered.

Confidentiality
Your medical information is strictly confidential. We will not release it to anyone without your written consent. If you want a copy of your records sent to another physician we will require a written authorization from you.
Prescription Refills
Please request refills of your prescriptions at the time of your appointment. Dr. Barber and Dr. Burbidge will prescribe enough medication to allow you to safely reach your next appointment.
We do not refill prescription over the telephone nor by fax, as an accurate diagnosis is possible only with an office visit
